URGENT DEADLINE – FRIENDLY REMINDER TO MEMBERSHIP
Cowessess First Nation invites all membership eligible for the Per Capita Distribution to please contact our office with your updated address. We will forward an application to you or you may download the Application – Form 1 or Form 2 or Form 3, whichever applies to you. Please complete the Form with two (2) pieces of identification and return to our office as follows:

Parents, if you have children under the age of 18, the funds will be held in trust until they turn 18. They must apply on their own behalf and fill out Form 1. **PLEASE DO NOT APPLY ON THEIR BEHALF**
Please remember once you turn 18 years of age, you have one (1) year to apply for your trust dollars or will be forfeited if you miss the deadline. Please submit clear copies of identification along with the completed Form 1 application.
Legal or Property Guardians of members turning 18 must complete Form 2 along with clear legible photocopies of both the adult and guardian. A copy of court documents proving guardianship will also be required with the application. You have one (1) year to apply after the member turns 18 years old.
If you are applying on behalf of a deceased member, Form 3 will be required along with a copy of the death certificate.
Please note ** In accordance with Section 5.8 of the Trust Agreement, the time has now ended for adults to apply for the Flood Claim Per Capita Distribution. The three (3) year timeline was from May 16, 2013 to May 16, 2016.
